Thanks to One Fat Frog Restaurant Equipment for sharing this information.  This checklist of items is from the 2008 May 6 DBPR Form HR 5021-012 entitled “Division of Hotels and Restaurants Licensing Information Checklist, Florida Department of Business & Professional Regulation”

Form initially needs to know what type of request you are making:  opening, change of ownership, district/ inspector #, date.   Then there is a series of items that will need to be reviewed in person with the “person in charge”.  In brief here is that list of items the Florida Division of Hotels and Restaurants will look at:

  • employee health policy and foodborne illnesses
  • risk factors and food code interventions
  • no bare hand contact with read-t-eat
  • Date marking for ready-t0-eat
  • Time as a public health control
  • Food processes- heating, cooling, etc
  • Required equipment and calibrated
  • certified food mgr requirements
  • food employee training requirements
  • shellfish advisory (if applicable)
  • parasite destruction freezing records for non-exempt raw and undercooked fish
  • shellfish tag maintenance
  • Hospitality Education Program (HEP) training availability
  • Choking poster
  • Chem strips for sanitizer
  • boil water notice
  • Menu items and misrepresentation
  • handwashing signs
  • hot and cold water at sinks
